Introduction to the Pre-Participation Exam (PPE)
PLNU Sports Medicine requires all Student-Athletes to complete an annual Pre-Participation Exam (PPE) to fulfill medical eligibility requirements as set forth by the NCAA and PLNU Athletics. The PPE process includes several different components: a physical exam, medical history questionnaire, concussion screening, mental health screening, documentation/forms, and sickle cell testing. Each Student-Athlete is required to complete every component prior to satisfying medical eligibility. Incomplete PPE’s will result in medical ineligibility and the Student-Athlete will not be allowed to practice or compete until all necessary components have been completed. Each component to the PPE has been outlined below. The Student-Athlete is responsible for all information contained in the sections below. We recommend that you have a parent or guardian review this information with you. If you are under 18, a parent or guardian must review and sign each form in addition to your signature.Â
Pre-Participation Exam
Every Student-Athlete is required to complete a physical exam and medical history questionnaire performed by a licensed physician (MD, DO, NP, PA-C) PRIOR TO ARRIVAL ON CAMPUS. Forms signed by other providers, including but not limited to Doctor of Chiropractic (DC), Doctor of Physical Therapy (DPT/PT), Naturopath, etc. will not be accepted regardless of your home state legislation. This exam must be completed on the
PLNU Sports Medicine PPE Form (PDF) and signed and stamped by the physician. This form will be reviewed by PLNU’s Team Physicians and will determine the Student-Athlete’s medical eligibility to participate in practices or competitions. PLNU Sports Medicine staff reserves the right to make final decisions regarding the participation status of all Student-Athletes.
HEALTHY ROSTER
PLNU Sports Medicine utilizes an Electronic Medical Records (EMR) system called HEALTHY ROSTER for the maintenance and tracking of Student-Athletes’ various medical documentation. All online documentation, forms, etc for each Student-Athlete will be collected and maintained through HEALTHY ROSTER during your time here at PLNU. Concussion and Mental Health Screening
All concussion and mental health screenings will be completed on PLNU’s campus prior to Student-Athlete participation in PLNU athletics. You MUST review the NCAA Concussion Fact Sheet prior to reporting for your concussion screening. You will review and sign this form on ATS.Â
Sickle Cell Test
The NCAA requires that all Division II Student-Athletes be tested for sickle cell trait. The following will provide further information for those who need to get tested, as well as how to access results for previously completed tests. More detailed sickle cell information can be found on the NCAA Sickle Cell Fact Sheet and at
NCAA.org.Â
ALL Student-Athletes must have submitted the results of their sickle cell test prior to any form of participation in athletics. There are 3 options available for obtaining the sickle cell test results:Â
1. If you were tested at birth or have previously had a sickle cell test completed.
2. If you were not tested and would like to be tested at home.
3. If you would like to be tested in San Diego.Â

After you have received your results you need to upload a copy of the documentation into your HEALTHY ROSTER PROFILE. Refer to the HEALTHY ROSTER page for further instructions.
Under 18?Â
All Student-Athletes under the age of 18 at the time of their Pre-Participation Exam MUST have all forms signed by a parent or guardian as well as by the Student-Athlete. This includes the PPE form as well as all online documentation within the
HEALTHY ROSTER system.
